Who we are
Cola Clinic (colaclinic.com) is the professional website of Mark D. Colafranceschi, D.C., a licensed chiropractic physician in Scottsdale, Arizona (Arizona License #009154). In this policy, “we” and “us” mean Cola Clinic and Dr. Mark Colafranceschi.

Information you give us
The intake form. When you complete the Cola Clinic intake, you provide:
- your name, email address and cell phone number
- your age and date of birth
- your answers to the symptom questionnaire
- your health history
- how often you eat certain foods and drinks
- your consent to the intake terms
When you submit the form, your answers are sent by email to Dr. Mark at Drcola1010@gmail.com. The website does not keep a copy of your submission in its database. Standard email may not be encrypted end to end. If you would rather provide your intake another way, call 208-315-1010 before you submit.
Calls, texts and email. If you call, text or email us, we receive your phone number or email address and whatever you choose to tell us.
Information collected automatically
Analytics. This site uses Google Analytics to understand how visitors use it, such as which pages are viewed, how long visitors stay, the general location (city or region), and the type of device and browser. Google Analytics uses cookies to do this. It does not receive your intake answers. You can opt out with the Google Analytics opt-out browser add-on or by blocking cookies in your browser.
Hosting and security. Our web host, Hostinger, keeps standard server logs (such as IP address, browser type and the pages requested) to run, cache and protect the site.
Cookies. Besides analytics, the site may set cookies needed for it to work. You can block or delete cookies in your browser settings; the site will still work.
How we use your information

- to keep records of the care and recommendations we provide

What we do not do

When information is shared
We share information only when needed to provide our services (for example, the email and hosting providers that carry and store it), when you ask us to, or when the law requires it.
How long we keep information
Intake responses and related notes are kept as part of your clinical record for as long as needed to provide care and as long as Arizona law requires health records to be kept. Analytics data is kept according to Google Analytics settings.
Your choices

Children
This website is not directed to children under 18. A parent or guardian should complete the intake for a minor.
